GP practices begin working in nine Primary Care Networks in west Kent

GP practices in West Kent are beginning to work together in nine Primary Care Networks (PCN) instead of seven clusters.

A PCN consists of groups of general practices working together, and in partnership with community, mental health, social care, pharmacy, hospital and voluntary services in their local area, to offer more personalised, coordinated health and social care to people living in their area.

Typically the primary care networks serve populations of between 30,000 and 50,000 people, although some are either smaller or larger, where this makes sense for local needs.

West Kent’s PCNs are similar to the seven clusters that practices have been working in and will serve populations of between approximately 33,000 and 81,000.

The change has taken place in line with NHS England’s Long Term Plan, which required all GP practices across the country to form networks.
